As of 3rd July 2017 our practice list will be open to patients who are new to the Witham area, and patients wishing to transfer from other surgeries within Witham. 

For your information:

If you are looking to register with us because you cannot get an appointment at your present surgery, you will find that we are in a similar position.

If you have been removed from your present surgery because you did not attend your appointments, and failed to cancel them, we too have a policy that will remove you from our books should you not attend your booked appointments.

The Practice Manager may contact you should there be any concerns with your reason for leaving your present surgery.

If the above are not the reasons why you are leaving your present surgery, then you are welcome to join our practice.

New Patient Registration

If you wish to register with the practice you will be asked to fill in the appropriate forms. Please ensure that these are filled in correctly and completely, if not there could be a delay in your registration.

You will be offered a new patient check with the doctor / nurse to ensure your past medical history and medication is recorded on our computer. If you do not make an appointment you will not be able to have any medications or appointments. This will enable us to assist in your care until your full medical records arrive.

You may express a preference of practitioner when booking your appointment.

Change of Personal Details

If you change your name, address or contact details (such as your telephone number or e-mail address), please let us know as soon as possible, so that we can update our records. If you are receiving treatment at a hospital it might also be a good idea to inform them of any changes so that both our records are consistent when we communicate with them.
